What is a DPC?

A one-time enrollment fee of $100 per household is charged immediately after enrollment is processed.  
There is a 3 month commitment and you will be charged for all 3 months prior to your first visit. This charge will go through on the 1st of the month that your establishing care appointment is scheduled. These fees are non-refundable. After your first 3 months, you can pay monthly, quarterly, every 6 months, or annually.
